Home Health Nurse

Home Health Registered Nurse (RN) – Full-Time | Flexible Schedule | Guaranteed 40 Hours

This role supports a PACE program that enables seniors to age in place by providing coordinated medical care and community-based services. The Home Health RN brings vital care directly into participants’ homes, improving access and outcomes.

Locations available: San Jose, Long Beach, North Hollywood

Key Responsibilities:

  • Perform in-home nursing assessments to determine equipment and care needs
  • Actively contribute to interdisciplinary team (IDT) meetings and participant care plans
  • Provide and document skilled nursing interventions, including administering medications and executing treatment plans
  • Educate participants, families, and caregivers on conditions, self-care, and prevention strategies
  • Assist with home health scheduling in partnership with management
  • Coordinate with physicians and other team members for smooth transitions across care settings

Qualifications:

  • Graduate of an accredited nursing program
  • Active, unencumbered RN license
  • 1+ year of clinical nursing experience, preferably with elderly or frail populations
  • Home health agency experience is a plus
  • Must have reliable transportation
